HEALTHCARE WORKER BACKGROUND CHECK/FINGERPRINTING
All applicants must submit to a Criminal Background Check initiated by the school prior to beginning of the program, in compliance with the Health Care Workers Background Check Act to be accepted into the program. It is required to obtain employment as a CNA. Applicants must attend orientation day if fingerprinting has not been done. If fingerprinting has already been done, the fee is waived. Applicants with a conviction will not be accepted into the program. A waiver will be needed for the application to be approved. In the event when a conviction** is found prior or during the program, the applicant must be withdrawn and will not be able to attend until waiver is granted.
REFUND POLICY
Spectrum Nurses Training School has adopted the following refund policy, which is in accordance with the State of Ilinois Board of Education. Spectrum Nurses Training School will comply with the policy that applies at the time of the student's enrollment. A copy of the specific applicable refund policy will be provided to the student upon enrollment and is an integral part of the enrollment agreement.
When a student gives a written notice of cancellation, then the school will provide a refund in the amount of at least the following:
1.
- When notice of cancellation is given before midnight of the business day after the date of enrollment but prior to the first day of class, all application-registration fees, tuition, and any other charges shall be refunded to the student;
- When notice of cancellation is given after midnight of the fifth business day following acceptance but prior to the close of business on the student's first day of class attendance, the school may retain no more than the application-registration fee which may not exceed $150 or 50% of the cost of tuition, whichever is less;
- When notice of cancellation is given after the student's completion of the first day of class attendance, but prior to the student's completion of 5% of the course of instruction, the school may retain the application-registration fee, an amount not to exceed 109% of the tuition and other instructional charges or $300, whichever is less.
- When a student has completed in excess of 5% of the course of instruction the school may retain the application-registration fee but shall refund a part of the tuition and other instructional charges in accordance with the following: i. After 5% of the course of instruction, but within the first week of classes the school shall refund at least 70% of the tuition; During the first 25% of the course, the school shall refund at least 55% of the tuition; During the second 25% of the course the school shall refund at least 30% of the tuition; In cases of withdrawal after 50% of the course, the school may commit the student to the remaining obligation of the tuition.
2. A student, who on personal initiative and without solicitation enrolls, starts, and completes a course of instruction before midnight of the fifth business day after the enrollment agreement is signed, is not subject to the cancellation provisions.
3. Applicants not accepted by school shall receive a refund of all tuition and fees paid within 30 calendar days after the determination of non-acceptance is made.
4. Application-registration fees shall be chargeable at initial enrollment and shall not exceed $150 or 50% of the cost of tuition, whichever is less.
5. Deposits or down payments shall become part of the tuition.
6. The school shall mail a written acknowledgment of a student's cancellation or written withdrawal to the student within 15 calendar days of the postmark date of notification. Written acknowledgments are not necessary if the refund has been mailed to the student within 15 calendar days.
7. All student refunds shall be made by the school within 30 calendar days from the date of receipt of the student's cancellation.
8. A student may give notice of cancellation to the school in writing. The unexplained absence of a student from a school for more than 15 school days shall constitute constructive notice of cancellation to the school. For the purpose of cancellation the date shall be the last day of attendance.
9. The school shall refund all monies paid to it in any of the following circumstances:
- The school did not provide the prospective student with a copy of the student's valid enrollment agreement and a current catalog/bulletin.
- The school cancels or discontinues the course of instruction in which the student has enrolled.
- The school fails to conduct classes on days or times scheduled, detrimentally affecting the student.
WITHDRAWALS
Students who choose to withdraw from school must do so in writing to the director of the school. If a refund to the school is applicable, the school has up to 30 calendar days from the date of the notification to provide the student with their refund.
NOTICE TO STUDENT
1. Do not sign this agreement before you have read it or if it contains any blank spaces.
2. This agreement is a legally binding instrument and is only binding when the agreement is accepted, signed, and dated by authorized official of the school or the admission officer at the school's principal place of business. Read all pages of this contract before signing.
3. You are entitled to an exact copy of the agreement and any disclosure pages you sign.
4. This agreement and the school catalog constitute the entire agreement between the student and the school.
5. Any changes in this agreement must be made in writing and shall not be binding on either the student or the school unless such changes have been approved in writing by authorized official of the school and by the student's parent or guardian. All terms and conditions of the agreement are not subject to amendment or modification by oral agreement.
6. The school does not guarantee the transferability of credits to another school, college, or university. Credits or coursework are not likely to transfer; any decision on the comparability, appropriateness and applicability of credit and whether credit should be accepted is the decision of the receiving institution.
STUDENT'S RIGHT TO CANCEL
The student has the right to cancel the initial enrollment agreement until (time) of the (X") business day after the student has been admitted. If the right to cancel is not given to any prospective student at the time the agreement is signed, then the student has the right to cancel the agreement at any time and receive a refund paid to date within 2 days of cancellation. Cancellation should be submitted to the authorized official of the school in writing.
STUDENT ACKNOWLEDGEMENTS
1. I hereby acknowledge receipt of the school's catalog, which contains information describing programs offered, and equipment or supplies provided. The school catalog is included as part of this enrollment and I acknowledge that I have received a copy of this catalog.
2. I have carefully read and received an exact copy of this enrollment agreement.
3. I understand that school may terminate my enrollment if I fail to comply with attendance, academic, and financial requirements or ifI fail to abide by established standards of conduct, as outlined in the school catalog. While enrolled in the school, I understand thatI must maintain satisfactory academic progress as described in the school catalog and that my financial obligation to the school must be paid in full before a certificate or credential may be awarded.
4. I hereby acknowledge that the school has made available to me all required disclosure information listed under the Consumer Information section of this enrollment agreement.
5. I understand that the school does not guarantee transferability of credit and that in most cases, credits or coursework are not likely to transfer to another institution. In case where transferability is guaranteed, Spectrum Nurses must provide me copies of transfer agreements that name the exact institution(s) and include agreement details and limitations
6. I understand that the school does not guarantee job placement to graduates upon program completion.
7. I hereby grant the Spectrum Nurses Training permission to use my likeness in a photograph, or other digital media ("photo") in any and all of its publications, including web-based publications, without payment or other consideration.
The student acknowledges receiving a copy of this completed agreement, the school catalog, and written confirmation of acceptance prior to signing this contract. The student by signing this contract, I understand the terms and conditions, and agrees to the conditions outlined in this contract. It is further understood that this agreement supersedes all prior or contemporaneous verbal or written agreements and may not be modified without the written agreement of the student and the School Official. The student and the school will retain a copy of this agreement.
